Introduction
MONT Oral & Maxillofacial Surgery is a surgeon-owned private practice dedicated to providing hospital-caliber surgical care in a patient-centered environment. Both Dr. Kim and Dr. Farag hold active appointments at NewYork-Presbyterian Hospital / Weill Cornell Medical College and bring that academic standard to every procedure performed in our office.
Whether you've been referred by your dentist or found us on your own, our team is here to guide you through your treatment with clear communication, thorough planning, and genuine care at every step.

Getting Started
Scheduling a Consultation
To schedule a consultation, call our office at (201) 212-5837 or submit an appointment request online. Our team will confirm your appointment date and time and send you any forms needed in advance.
Most patients are referred by their general dentist or orthodontist. If you have a referral, please bring it to your appointment. We also welcome self-referrals for most procedures.
What to Bring
- Photo ID and insurance card(s)
- Referral letter from your dentist or doctor (if applicable)
- List of all current medications and supplements
- Any X-rays or imaging you've already had taken
- List of medical conditions, allergies, and prior surgeries
- Insurance authorization or pre-authorization (if required by your plan)
If you are under 18, a parent or legal guardian must be present at the consultation and on the day of any procedure.

What to Expect
Your First Visit
Your initial consultation typically lasts 30–60 minutes. A member of our team will review your medical and dental history, and we may take X-rays or a 3D CBCT scan in our office if needed.
You'll meet with your surgeon to discuss your concerns, review findings, and go through treatment options in plain language. There's no pressure — the goal of the first visit is to make sure you understand your situation and feel confident about the path forward.
Before you leave, we'll provide a clear treatment plan, an estimate of fees, and answers to any questions you have. You'll know exactly what to expect.
Check-In
Complete any remaining paperwork and verify insurance.
Medical History Review
A team member reviews your health history, medications, and allergies.
Imaging
X-rays or CBCT scan taken if needed — usually done in-office.
Surgeon Consultation
Your surgeon examines you, explains findings, and discusses treatment options.
Treatment Plan
You receive a written summary of the recommended treatment and next steps.
New Patients
Patient Registration
New patients are asked to complete registration forms before their first appointment. These forms capture your health history, insurance information, and consent for treatment.
We will email or text you a link to complete forms electronically before your visit. If you prefer paper forms, they'll be available at the front desk when you arrive — please plan to arrive 15 minutes early if completing them in-office.

Fees & Insurance
Financial Information
We accept most major dental and medical insurance plans. Coverage varies significantly by plan, so we encourage you to contact your insurance company before your visit to understand your benefits for oral surgery procedures.
Our team will verify your insurance and provide a fee estimate before treatment begins. For procedures requiring prior authorization, we'll submit the request on your behalf.

Surgical Instructions
Surgical Instructions
The following instructions apply to most procedures at our office. Your surgeon will provide personalized instructions at your pre-operative appointment. If anything is unclear, please call us before your surgery date.
Pre-Operative Instructions
If You Are Receiving IV Sedation or General Anesthesia
- —Do not eat or drink anything (including water) for at least 8 hours before your procedure. This is a strict safety requirement — your procedure will be rescheduled if this is not followed.
- —Arrange for a responsible adult to drive you to and from the office and stay with you for the remainder of the day. You may not drive or operate machinery for 24 hours after sedation.
- —Take your regular medications the morning of surgery with a small sip of water unless your surgeon has told you otherwise.
- —Wear loose, comfortable clothing with short or roll-up sleeves. Avoid jewelry, nail polish, and contact lenses.
- —Do not smoke for at least 12 hours before your procedure.
For All Procedures
- —Alert our office immediately if you develop a cold, fever, or illness before your appointment.
- —Inform us of any changes to your medications or health status since your consultation.
- —Fill any prescribed pre-operative medications (antibiotics, anti-inflammatories) before the day of surgery.
- —Plan for rest after your procedure — most patients benefit from taking the remainder of the day off.
